B. Graczyk, Inc. Terms And Conditions

B. GRACZYK, INC. feels such a strong loyalty and commitment to its customers and to its manufactured instruments that we offer you this guarantee.

LIFETIME WARRANTY: All B. GRACZYK, INC. Stainless steel instruments are unconditionally guaranteed against manufacturing defects. Any instrument found to have a manufacturing defect will be repaired or replaced free of charge.

REPAIRS

B. Graczyk, Inc. provides repair services for all
B. Graczyk, Inc. instruments, as well as instruments from other manufactures. Repaired instruments will be sent back to the customer within 14 business days of receipt by
B. Graczyk, Inc.

When an instrument needs to be repaired please use the following procedure.

1. Clean instruments thoroughly, then sterilize before
    shipping them to B. Graczyk, Inc.

2. To ship instruments, use a sturdy shipping box with soft
    packaging material to protect your instrument.

3. Use tip protectors and individually wrap micro and
    delicate instruments.

Return instruments for repair to B. Graczyk, Inc.

PRICES

B. GRACZYK, INC. will make every effort to maintain its prices; however, all prices are subject to change without notice.

ACCEPTANCE

All orders are subject to approval and acceptance of home office.

QUOTATIONS

B. GRACZYK, INC. welcomes the opportunity to submit price quotations on large instrument bids. Price quotations on all B. GRACZYK, INC. products will be valid for a period of 30 days unless otherwise specified in writing.

PAYMENT TERMS

Our payment terms are net 30 days.

PACKAGING AND SHIPMENT

All shipments are FOB or US Mail, and charges will be added to your invoice.

All visible damage or apparent shortages must be noted on the carrier's delivery document.

The customer shall inspect all items upon arrival and report any error in shipment or damage at once (within 5 days) by calling B. GRACZYK, INC. Customer Service at 630-871-2440.

RETURN MERCHANDISE POLICY

All returns must be accompanied by a Return Material Authorization Number. This number is available by calling our Customer Service Department. Invoice number, order number, and date of shipment must be supplied in order to obtain authorization.

Merchandise return for credit will be accepted under the following terms and conditions.

1. Authorized merchandise returned within 30 days after
    our date of shipment, providing the product is received
    by us in perfect condition and is in its original package,
    will receive full credit.

2. Authorized merchandise returned within 30-90 days after
    shipment will be subject to a 15% restocking charge.

3. Returns after 90 days are not acceptable.

4. Any product returned which is not in its original package
    is subject to a restocking charge of 25% according to
    its condition.

The following items are non-returnable for credit except those resulting from a B. GRACZYK, INC. error.

a. Sterile Products.

b. Custom made instruments.

c. Custom etched instruments.

d. Damaged or abused instruments.

e. Discontinued products.

f. Custom-made products.

GENERAL

B. GRACZYK, INC. is not responsible for typographical errors made in any of its publications or for clerical or stenographic errors made in sales orders, acknowledgements or preparation of quotations. All such errors are subject to correction.

The "Terms and Conditions" as set forth above shall govern all transactions. Any inconsistent terms wherever set forth and whether written, oral, or implied shall not be binding on B. GRACZYK, INC. unless agreed to in writing by B. GRACZYK, INC.
